EXCLUSIONS & LIMITATIONS
Our in-house dental plan is not an insurance plan, it has been designed to provide a way for our patients to obtain the dental care they need at a more affordable rate. Benefits are available only with Dr. James Amir and his team for treatment within their realm of expertise. Over-the-counter products are not included. The fees of any specialist that we may refer to are not part of this plan. Our plan may not be combined with any other discounts or insurance, including instances that involve a lawsuit due to injury. Plan fees are not refundable. No refunds are given if patients do not use their plan benefits during their enrollment period. Unused benefits are not transferable to another person. All dental services must be completed during the enrollment period. If enrollment expires before treatment is completed, coverage is no longer available. Given the substantial discounts offered under this plan, payment for all single-visit services is due at the time of service. Any service that requires multiple appointments may be paid with a 50% deposit at the start of treatment, with the balance being paid in full at completion of the service. If you choose to extend your payment by paying through CareCredit, or any similar healthcare financing merchant, and the discount changes from 15% to 5% due to the merchant’s fees. Plan fees and discount rates are subject to change annually.
